There are three ways to add notes to a child record. You can add Program Notes directly to an assessment, notes to an item in an assessment, or Head Start notes to the child record.
Adding Program Notes to an Assessment 1. To add a note directly to an assessment, choose Child Administration > Search Child. When the Child–Search and Manage page opens, use the Search features to locate the child record. Click

Adding Notes to an Item in an Assessment 1. To add a note directly to an item in an assessment, choose Child Administration > Search Child. When the Child–Search and Manage page opens, use the Search features to locate the child record. Click
2. When the Assessments page opens, click the date to open the assessment.
3. Click Item Details and select the domain or subdomain to which you want to add the note. Scroll to the bottom of the page and click
4. When the Edit page opens, locate the item and click
Note: A score must be entered for the item before the program saves the note.
Adding a Head Start Note to a Child Record 1. To add a Head Start note to a child record, choose Child Administration > Search Child. When the Child–Search and Manage page opens, use the Searchfeatures to locate the child record. Click
